Program Description
Clinical documentation is more than a record of what happened in session; it supports ethical care, medical necessity, billing accuracy, continuity of treatment, and risk management. This program focuses on documentation practices that help therapists write notes that are clinically meaningful, defensible, and aligned with professional responsibilities. Participants will explore common documentation gaps, medical necessity, treatment rationale, functional impairment, and how notes can better support the services provided.

After this program, participants will be able to:
Identify key documentation elements that support medical necessity and ethical clinical care.
Describe common documentation gaps that may create billing, audit, or clinical risk.
Develop one strategy for improving documentation consistency in their practice.
